In his words
The commonest mistake I see is treating a blood test instead of a patient. A positive ANA in someone with no symptoms is not lupus, and a negative RA factor does not rule out rheumatoid arthritis.
In inflammatory arthritis there is a window in the first few months where treatment prevents damage that cannot later be undone. Almost everything about the long-term outcome is decided in that window.
Capillaroscopy takes five minutes and no needles, and it can tell you years in advance whether cold-sensitive fingers are harmless or the first sign of a connective tissue disease.
Remission is a realistic goal for most patients now, not an exceptional outcome. What it needs is an accurate diagnosis, treatment started early, and monitoring that does not stop when the patient feels well.
