What happens during the consultation

Most of a first visit is conversation. When your symptoms started, how they have changed, what makes them better or worse, what has already been tried, and what else has been going on in your body — that history is what narrows a hundred possible diagnoses down to two or three.

The examination that follows looks at more than the joint that hurts. Rheumatic disease is systemic, so skin, nails, eyes, mouth, chest and muscle strength all get checked when they are relevant.

Only then do tests get ordered — and deliberately few of them. Autoimmune blood tests are easy to over-order and easy to misread; a positive result in the wrong context causes more harm than not testing at all.

What to bring

  • Previous prescriptions and discharge summaries, in date order if possible
  • All recent blood reports, including any autoimmune tests already done
  • X-rays, MRI or CT films and their reports — the images, not only the reports
  • A written list of every medicine and supplement you currently take, with doses
  • A short note of when symptoms began and how they have changed

If reports are on your phone, that is fine. If you have been to several doctors, bring everything — including the tests that came back normal, because a normal result at a known date is useful information.

How long it takes

Allow more time than a routine OPD visit. A first rheumatology consultation involves a long history, a physical examination and a discussion of the plan, and it is not something to rush.

Bringing someone with you

If your symptoms include weakness, significant pain or memory of the timeline is difficult, bring a family member. They often remember dates and earlier treatments more accurately than the patient does.

Follow-up

Most rheumatic conditions are managed over years, not weeks. A first visit usually ends with a plan for tests and a review appointment, and treatment often begins before every result is back if the clinical picture is clear enough.
